it's not open source, but apple's Preview.app (part of OSX) knows how to deal with forms, allowing users to fill them out and print them. it's not perfect, but it worked well enough for me to do expense reports at my last job- i just had to write the total in by hand after printing it (apparently the official adobe reader software on windows was able to do the math automatically.)
