I've had great success with Switchvox (http://www.switchvox.com/).  It
is Asterisk-based and the company was recently purchased by Digium,
the primary sponsor of the Asterisk project.

I've never had a problem with it and it has saved me tons of time in
setup and maintenance.  Before that I setup my own custom Asterisk
setup, but it was just too hard to maintain because it was a lot of
config file editing.

We use the system on a voice T1 from PAETEC, but that's only because
we are still under contract with them.  I'm sure there are plenty of
VoIP providers that can beat Embarq's prices so long as you have a
solid internet connection.
