I use vitelity and teliax as voip providers with a homebrew asterisk 
box.  Costs me about $5 for the DID's I have and only fractional pennies 
for when I use the phone.
If you want an easy asterisk open source solution project look at 
Elastix (trixbox is another one, but a huge voip solution provider 
called Fonality bought them).
