I've been using Vonage for the past year plus. One voice line, one fax 
line (which might or might not be usable for voice), and free long 
distance to US, Canada, and much of Western Europe. Call forwarding, 
caller ID, and voice mail come with at no extra charge.

The only bug I've had since day one has been flaky performance by 
ComCast (using old Adelphia hardware) to deliver cable service; never a 
problem with Vonage, their hardware, or VOIP itself.

Pricing starts at $25/month (plus tax). I'm on the small business plan 
and pay about $38/month, including taxes.
