We use Asterisk for DR comm in our satellite comm trailers, some county health depts as their primary PBX and at HQ for the service desk. I've used it as a home user as well. I have a friend who runs a pc repair and consulting business and he uses it for all his business calls as well (only 3 or 4 lines for him)
